GETTING
APPOINTMENTS:
If you would like an appointment, please call us today at
817.433.5160. If you have need to contact us by fax you may
do so at 817.433.5161, or by email here. In your email, it is
helpful if you include possible appointment times.
BEFORE YOUR APPOINTMENT:
Before your INITIAL visit, we would like you to complete a
medical history report (time permitting), which you can
print from the PATIENT FORMS
page. Ideally,
you would return the form to us by mail or fax at least
a few days before your appointment.
Before any follow up visits, we would like you to complete
the FOLLOW-UP VISIT questionnaire which we give you in the
office. This paperwork gives us a great idea of your needs
for the visit and enables us to serve you better.
HOURS:
Appointments are scheduled from 8:00 AM to a final
appointment of 4:00 PM Monday, Wednesday and Friday. We
close by 3:30 Tuesday and Thursday. No Saturday or Sunday
appointments are expected at this time. Most new
appointments will be 30 to 60 minutes, and follow-up
appointments will be 20 to 30 minutes. These appointment
times are longer than many other internal medicine
practices.
HOSPITAL NEEDS:
I admit patients to Harris Methodist Southwest. I do not
have privileges at other inpatient facilities. Unless I am
out of town, I admit my own patients when hospitalization
is necessary.
